How to make an appointment

To make an appointment, click on the red “Make An Appointment” button at the top of this page or below. Then click on the “New Patient Intake Form,” and complete the following steps on the form.

Once this is completed our office will verify your testing benefits and possible hearing aid coverage. Once your insurance information has been verified you will receive a call to schedule an appointment.
